Revisional bariatric surgery (after failed previous bariatric surgery)

Patient selection: Some patients who had prior bariatric surgery and who experienced insufficient weight loss (or weight regain) and/ or those who developed complaints after their previous bariatric surgery can be offered good alternative bariatric surgical solutions.
Procedure: It depends on the previous type of surgery and the complaints of the patient which revisional bariatric surgery would be the best solution for that specific patient.
These revisional bariatric procedures require even more so a very experienced bariatric surgeon.

  • After previous Sleeve:
    • In case of post-sleeve weight: usually a revision to a mini-bypass or SADI bypass is usually performed.
    • In case of post-sleeve severe acidity reflux: a classic Roux-en-Y gastric bypass is usually performed.
  • After previous Gastric Bypass
    • In case of post-Roux-en-Y gastric bypass weight regain: a further distalisation of the RY-bypass with pouch revision is usually performed (possible with banding of the bypass).
  • After previous Lap Band
    • In case of post Lab Band weight regain: a mini-bypass is usually performed, unless if there is severe gastric acidity reflux as in that case a revision to a classic RY-gastric bypass is more ideal.
  • After previous Mini-Bypass (one-anastomosis gastric bypass)
    • In case of post mini-bypass weight-regain: a revision to a SADI bypass is a good option.
    • In case of post mini-bypass: biliary reflux: a revision to a distal RY-gastric bypass is a good option to (also) reduce acidity reflux.
