Haemorrhoids

General Surgery

Haemorrhoids are blood-vessel piles in the anus that can cause itching or sometimes bleeding, and in some cases (when the vessel is thrombosed) pain.

Causes

Aging, chronic constipation or diarrhoea, obesity, pregnancy, prolonged sitting, straining at defecation or micturition, rectal cancer, portal hypertension.

Symptoms

Itching, rectal bleeding and discharge, anal tissue protruding out from the anus, and usually haemorrhoids are painless unless the haemorrhoid is complicated (thrombosed, ulcerating or necrotic or when in combination with an anal fissure.

Diagnostics

Proctoscopy and sigmoidoscopy/ or colonoscopy to exclude at rectal mass.

Treatment

  • Conservative treatment: Warm sitz baths (2-3x/day), high fibre diet, drinking 2 litres/day, medical fibres (movicolon), anal oitments (containing hydrocortisone or lidocaine).
  • Rubber band ligation or laser therapy for minimal haemorrhoids.
  • Haemorrhoidectomy (resection of haemorrhoids) is occasionally used for large/ prolapsing haemorrhoids
  • (stapler resection is less recommended as it can harm the anal sphincter)

Risks of the surgery

Bleeding, pain, stenosis, anal fissure, temporally urine incontinence, scarring reaction.
